Nurse Case Manager/Utilization Review

We are seeking a dedicated and compassionate Nurse Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive care coordination and support to patients in a hospital setting. Duties:
- Conduct patient assessments and develop individualized care plans
- Collaborate with healthcare professionals to ensure appropriate treatment plans
- Monitor patient progress and adjust care plans as needed
- Coordinate discharge planning and ensure smooth transitions of care
- Advocate for patients' needs and rights
- Maintain accurate and confidential medical records
- Provide education and support to patients and their families
- Comply with HIPAA regulations and maintain patient privacy

Requirements:
- Bachelor's degree in nursing or related field (preferred but not required)
- Previous experience in case management or related healthcare role
- Strong knowledge of hospital procedures and protocols
- Familiarity with ICU and ER settings
- Excellent communication and interpersonal skills
- Ability to multitask and prioritize responsibilities
- Proficient in medical record documentation
- Knowledge of patient assessment techniques and vital signs monitoring

Pay: $35.00 - $50.00 per hour
