Seeking a LPN or RN to serve our Medicare patient population as their Chronic Care Management Nurse Navigator. Join our team of nurses who perform care coordination for our patients and guests. You will be a critical part of our team in caring for our patients by developing trusted relationships with them and being proactive in their health to improve their quality of life.

When succeeding in this role, you will develop relationships with a specific group of geriatric patients and their families by influencing directly their healthcare and giving them the best possible experience at our office.

Responsibilities: Triaging and managing a select number of Medicare patients from our practice involving routine check-in calls and managing needs as they arise; prescription management and education, callbacks or portal messages regarding lab and imaging results, scheduling preventive services and educating on the importance of these procedures, active management of chronic conditions to reduce hospitalizations, ED visits and readmissions, facilitate advanced care planning discussions, identify and address social needs as they arise.

Qualifications: LPN or RN active license. Compassion and love for the geriatric patient population. Prior nursing experience preferred in care coordination. Preferred experience in SNF, assisted living facilities or primary care setting.

Skills: Proficient in electronic medical records, computer skills such as Microsoft Word. Excellent communication/customer service skills. Team player.

Pay rate: negotiable based on prior experience

Must be COVID Vaccinated. Annual Influenza Vaccine required.

Job Type: Part-time
