Position Summary/Responsibilities:

The Case Manager - RN will function in the role of a nurse home visitor in the nurse home visiting project. This position is responsible for coordinating, negotiating, procuring, and managing the care of complex patients and families to facilitate achievement of quality/cost outcomes. Works collaboratively with interdisciplinary staff, internal and external to the organization. Participates in the development of care plans to include short and long term goals related to clinical, psychosocial, and discharge planning issues. All cases of Perinatal/infant mortality will be reviewed by the Case Manager.

Additionally, the incumbent is the primary resource person for the healthcare provider to obtain information regarding current charges, reimbursement, and payer source limitations. The Case Manager functions in acute, sub-acute, and outpatient settings. Regular attendance is required to perform the functions of this position. Works independently with guidance from the Program Director, Assistant Supervisor and Manager of Medical Service Area. All duties will be conducted under the direct supervision of the (Nursing Supervisor) Manager of Medical Service Area, including departmental matters such as discipline, leave requests, etc.

The normal work schedule is Monday- Friday, beginning at 8:00 AM and concluding at 4:30 PM. Lunch breaks will be dictated by workload. Overtime may be required to meet assigned deadlines. Regular attendance is required to perform the functions of this position. Incumbent will maintain a customer-oriented awareness in acknowledging and responding to needs as they occur to assure that customer satisfaction is met at all times.

Minimum Qualifications:Applicant must have Maternal-Child or Home Health experience and meet one of the following requirements:

· Bachelor’s Degree in Nursing with three (3) years of Nursing experience of which one (1) of those years must have been in a clinical setting. Certified in adult, child and infant cardiopulmonary resuscitation or willingness to obtain certification. Possession of a current Louisiana RN license.

· A Diploma or an Associate Degree in Nursing with a minimum of three (3) years of Nursing experience of which one (1) of those years must have been in a clinical setting. Certified in adult, child, and infant cardiopulmonary resuscitation or willingness to obtain certification. Possession of a current Louisiana RN license.

Preferred Qualifications:

· Computer experience with database operations. Previous experience in case management, utilization review or related area.
